Transportation Consent

I give my consent for my child (children) to be transported and supervised by facility staff on field trips and to and from school. I understand the arrangements and believe that the necessary precautions and plans for the care and supervision of the children during trips will be taken. Beyond this, I will not hold the school or those supervising the trip responsible.
